Full Job Description

Job description:

Want a break from the fast-paced environment of nursing in a hospital setting, looking to branch out to further increase your nursing experience, or perhaps just moonlight for some extra income? Come apply and join our team!

Under the general supervision of the Administrator, the RN Supervisor assesses the need of our clients for personal care services according to established nursing processes of diagnosing, planning, intervention, and evaluation. Supervises the care of our clients living at home, trains and supervises personal care workers, works with a diverse population which includes individuals whose primary language is not English, collaborates with nursing team and clients’ primary care providers to establish a plan of care, consult with Director of Billing regarding clients’ PA, make referrals and follow-up. RN will be expected to travel outside of home community to provide training, in-home assessments, and supervision as needed.

Essential Duties:

Includes the following. Other duties may be assigned as needed:

Assess and evaluate the need for services and make referrals to other services as appropriate.
Secure written orders from the client’s physician. These orders are to be renewed once every three months unless the physician specifies that orders covering a period of time up to one year are appropriate or when the client’s needs change, whichever occur first.
Develop a Plan of Care giving full consideration to the client’s preference for service arrangements and choice of PCW, interpret the plan to the PCW, include a copy of the plan in the client’s health record, and review the plan at least every 60 days and update it as necessary.
Ensure services are performed according to a written plan of care. The plan shall be based on the registered nurse’s visit to the recipient’s home and shall include: (Review and interpretation of the physician’s orders; frequency and anticipated duration of services; evaluation of the recipient’s needs and preferences; and assessment of the recipient’s social and physical environment, including family involvement, living conditions, the recipient’s level of functioning and any pertinent cultural factors such as language.)
Review the plan of care, evaluate the recipient’s condition and supervise the PCW at least every 60 days. The review shall include a visit to the recipient’s home, review of the PCW’s daily worker’s daily written record and discussion with the physician of any necessary changes in the plan of care.
Promptly notify a client’s physician or other appropriate medical personnel and legal representative, if any, of any significant changes observed or reported in the client’s condition.
Develop appropriate time and service reporting mechanisms for PCWs and instruct the workers on their use.
Provide written instructions to PCW about the services to be performed and arrange for an appropriate person to demonstrate to the PCW how to perform the services.
Evaluate the competency of the PCW to perform the services.
Adhere to home visit schedule that is prepared monthly.
Use of online Personal Care Screen Tool (PCST) to correctly calculate client hours.
Review and sign off on PCW time sheets every payroll.
Perform monthly audits of clients’ medical records.
Educate clients and their families to ensure informed decision-making regarding their care.
Attend all required staff meetings and in-service trainings.
Comply with all OSHA, Medicaid, and HIPAA regulations.
Discharge clients as per DHS guidelines 105.17(1w)(b)(f)1-7.

Requirements:

Graduate of a nationally accredited Nursing program.
Current Wisconsin licensure as a Registered Nurse and minimum 3 year’s supervisory in personal care/home health services or related program. If no experience in personal care/home health, willing to train the right candidate.
Must pass drug test.
Must pass tuberculosis skin test.
Must pass caregiver background check.
Skills in leadership, organization, negotiation, conflict-resolution, and decision making.
Effective oral, written communication and interpersonal skills.
Basic keyboarding and computer skills.
Ability to work well under pressure and capable of meeting deadlines.
Ability to work independently and collaborate effectively as part of an interdisciplinary team.
Maintain detailed work schedule on the Intranet calendar.
Ability to maintain good working relationship with office staff as well as healthcare partners.
Ability to work with cultural diversity and with an interpreter.
Current WI driver’s license and reliable transportation.

Pay: $30.00 per hour
