Position: Full - Part Time or PRN -Physical Therapist One AL/IL facility

About the Opportunity
Our Home Health Organization is seeking a Physical Therapist to support patients and provide skilled physical therapy services in patients’ homes, in accordance with physician orders and established plans of care. You’ll collaborate with an interdisciplinary care team to improve patient mobility, safety, functional independence, and overall quality of life in the home setting.
Responsibilities:
- Perform initial and ongoing comprehensive physical therapy evaluations, including OASIS assessments as required.
- Develop, implement, and update individualized physical therapy plans of care in collaboration with physicians and the interdisciplinary team.
- Establish measurable goals based on patient functional status, rehabilitation potential, and safety needs.
- Provide skilled interventions to address strength, balance, gait, endurance, mobility, and pain management.
- Instruct patients and caregivers in therapeutic exercises, functional mobility, transfer training, and fall prevention strategies.
- Assess home environments and recommend modifications to promote safety and independence.
- Instruct patients in the safe use of assistive devices, durable medical equipment, and mobility aids.
- Supervise Physical Therapy Assistants (PTA) in accordance with state and federal regulations.
- Communicate regularly with physicians regarding patient progress and obtain updated orders as needed.
- Complete visit notes, progress notes, reassessments, and discharge documentation within agency-defined timeframes.
- Participate in discharge planning and complete Physical Therapy discharge summaries.
- Attend patient care conferences, in-services, and Quality Assessment and Performance Improvement (QAPI) activities as assigned.
- Maintain compliance with all agency policies, state regulations, and Medicare guidelines.
Qualifications:
- Graduate of a Physical Therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE)
- Current Physical Therapist license in the state of practice
- Minimum 1 year of institutional or clinical experience
- Home health experience preferred
- Supervisory experience with PTAs preferred
- Strong clinical judgment, organizational skills, and communication abilities
- Ability to work independently and manage PRN scheduling effectively
