Job Summary
The Integrated Behavioral Health (IBH) Program Manager will maintain primary accountability for the management and delivery of safe and effective patient care and for the day-to-day functions of Share Ourselves Integrated Behavioral Health program across all of our clinical locations. The areas of work for the Share Ourselves IBH Program Manager; include, clinical practice, integrated behavioral health program visioning, evaluation and development, program management, clinical oversight of licensed behavioral health clinicians and clinical supervision of trainees (if applicable) in a multi-clinic federally-qualified health center (FQHC). May perform other related duties as required within their scope of practice. The IBH Program Manager works in a team based model of care.
RESPONSIBILITIES
Clinical practice
- Facilitate interprofessional team meetings, which may include primary care clinicians, nurses, medical assistants, dentists, clinical pharmacists, case managers, referral coordinators, and community health outreach workers.
- Manage the care of patients in collaboration with other members of the health care team.
- Provide same day visits, warm handoffs, assessments, intervention, follow-up and care management.
- Provide high-quality behavioral health care to patients across the lifespan.
- Collaborate with Primary Care Clinicians.
- Participate in morning huddles with primary care clinicians, case managers, and back office clinical staff.
- Participate in case reviews once per month.
- Document progress notes in the EHR in a timely manner as required by the law and clinic guidelines.
- In collaboration with the care team, facilitate patients to services to support social determinants of health including supportive community services, in-home medical services, supportive housing, etc.
- Input and maintain patient information in the EHR system within Share Ourselves policy.
- Ability to work with patients via tele-health platform.
- 10-14 patient visits on average in an 8-hour day.
- Be accountable for maintaining quality integrated behavioral health services and meeting all regulatory, compliance and funding source standards while serving as a primary care team member.
- Ensure upkeep of necessary continuing education to maintain license & monthly compliance training as required by Share Ourselves.
Integrated behavioral health program visioning, evaluation & development
- Develop strategic organizational vision for integrated behavioral health in primary care with senior leadership
- Ongoing BHC in primary care workforce development strategy in partnership with clinic leadership
- Plan-Do-Study-Act (PDSA) change management approach to system and clinic quality improvement
- Develop and monitor integrated behavioral health policies, procedures, and standards of practice.
- Develop and monitor IBH clinical-operations workflow in collaboration with clinic management
- Support Share Ourselves Performance and Quality Improvement Plan and participate in discussion regarding department or program goals, data collection and/or analysis
- Responsible for ongoing tracking and review of documentation required for grants and other requirements as applicable. This may entail coordination of ongoing chart reviews, participation in various clinical meetings, paperwork tracking, and audit preparation.
- Evaluates effectiveness of program and services
- With EHR support team, develop EHR pull reports to track program effectiveness measures; like,
- Patients seen by each BHC in the last 3 months (Follow-up report)
- Percent of patients seen by BHC annually from assigned clinic panels (Population reach)
- Demographics of patients seen by BHC annually (Service equity & acceptability)
- Number of documented visits & care management notes a day for each BHC (Productivity)
- Percent of patients seen >5 times in last 3 months for each BHC (Care management tracking)
Integrated behavioral health program management
- Management of all behavioral health clinicians to include interviewing, hiring, mentoring, performance management, corrective action plans and schedules/time off management.
- Revenue cycle management for integrated behavioral health services
- Oversight of practice compliance for behavioral health clinicians in partnership with senior leadership, clinic leadership, HIPPA officer and others
- Internal and external communications regarding integrated behavioral health in primary care services
- Serve as an administrative liaison with outside agencies to promote behavioral health of patients. Facilitate referrals between Share Ourselves and other community-based programs and services.
- Provide periodic email communications to all BHCs.
- Provide periodic updates on the behavioral health program for all-staff communications
Clinical oversight & supervision
- Clinical oversight of BHCs integrated into primary care teams in Share Ourselves clinics
- Clinical supervision of trainees (or oversight of trainee clinical supervisors)
EDUCATION, LICENSURE & EXPERIENCE
- Master’s in Social Work, Mental Health Counseling, Psychology, or Marriage and Family Therapy
- Active clinical licensure in California (LCSW, LMFT, LPCC, or Psychology).
- 7+ years of progressive experience in behavioral health practicing in the Primary Care Behavioral Health (PCBH) model, as a BHC with at least 3 years in a leadership or program management capacity.
- 3-5 years of experience clinically supporting patients without a home and who may have substance use disorders preferred.
- Demonstrated expertise in implementing and managing the IBH model at scale, including experience leading multidisciplinary teams and managing industry‑standard patient panels.
- Proven experience launching or expanding behavioral health integration programs in medical or primary care settings.
- Strong understanding of California behavioral health regulations, payer requirements, and integrated care reimbursement models.
- Ability to inspire, mentor, and develop high‑performing clinical teams; strong track record of coaching clinicians and unlicensed staff.
- Capable of translating vision into executable plans; comfortable balancing long‑term strategy with day‑to‑day implementation.
- Deep understanding of evidence‑based behavioral interventions, safety risk assessments, and collaborative care workflows.
- Exceptional ability to build trust and collaborate across clinical, operational, and business functions.
- Comfortable operating in a fast‑paced, evolving environment and leading through change.
- Certified Addiction Treatment Counselor by CAADE, CADTP or CCAPP or comparable education, licensure and experience preferred
- Clinical supervision experience with trainees and licensed BHCs
- Desire to work with an ethnically diverse adult, geriatric and youth population.
- Cultural sensitivity and understanding of various populations including homeless, immigrants and LGBTQIAN.
- Knowledge of basic medical terminology, and psychiatric management medications.
- Program management experience
- Proficiency with Windows, Microsoft Suite (Word, Excel, Outlook, Teams, PowerPoint, Internet Explorer) and digital communication tools
- Proficient in EHR systems (Epic preferred)
- Bilingual in English and Spanish strongly preferred
- BLS required
- Valid State Driver’s License and vehicle insurance
