West Rest Haven is a community-owned and operated skilled nursing and rehabilitation facility dedicated to providing compassionate, high-quality care. Our 120-bed facility emphasizes a collaborative approach to patient well-being and recovery, fostering a supportive environment for both residents and staff.

Overview
A Registered Nurse or a Licensed Vocational Nurse is designated as a treatment nurse by the Director of Nursing Services for each shift and is responsible for providing specialized, compassionate nursing care to residents with chronic illnesses, disabilities, or age-related conditions requiring extended medical attention. This role combines clinical expertise with empathy to ensure residents’ physical, emotional, and psychosocial needs are met while collaborating closely with the interdisciplinary healthcare team.

The Treatment Nurses works in a collaborative, fast-paced environment that balances hands-on resident care with care coordination, documentation, and regulatory compliance. The role requires flexibility, resilience, strong organizational skills, and a commitment to continuous learning to meet the evolving needs of residents.

The Treatment Nurses play a vital role in enhancing residents’ quality of life by delivering skilled clinical care with compassion and dignity. Through attentive treatment, advocacy, and collaboration, they help residents maintain comfort, independence, and overall well-being in a long-term care setting.

Responsibilities

Monitor and document residents’ health status, vital signs, and responses to treatment
Administer medications, treatments, and therapeutic interventions as prescribed
Develop, coordinate, and implement individualized care plans with physicians, therapists, social workers, and other healthcare staff
Perform daily wound care and respiratory therapy treatments as ordered
Conduct weekly rounds with the wound care physician
Complete daily skin assessments
Maintain required respiratory certification courses and competencies
Manage time effectively while providing wound care and respiratory treatments for the entire facility
Provide emotional support, education, and guidance to residents and their families
Conduct ongoing health assessments and update care plans as needed
Respond promptly to emergencies and urgent resident needs
Maintain accurate and timely patient records in compliance with healthcare regulations and facility standards
Advocate for residents’ rights, dignity, and quality of care
Coordinate with interdisciplinary team members, including physical therapists, occupational therapists, and dietitians, to ensure comprehensive resident car

QUALIFICATIONS:

Current and valid Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license in the state of practice
Minimum of one (1) year nursing experience preferred, ideally in long-term care, geriatric care, rehabilitation, or chronic disease management
Strong clinical judgment and assessment skills
Ability to work independently and collaboratively within a team environment
Excellent communication, interpersonal, and documentation abilities
CPR and First Aid certification, or willingness to obtain upon hire
Basic computer proficiency and experience with Electronic Health Records (EHR)
Compassionate, patient-centered approach with the ability to thrive in a fast-paced healthcare setting
Must be able to speak, read and write the English language.

PHYSICAL / MENTAL / ESSENTIAL FUNCTION OF THE JOB:

Must be able to stand and walk throughout shift on hard floor and carpeting services to work with residents.
