Job Summary:

Job title: Admission Nurse (RN)
Job type: Full time
Location: Rochelle, IL. and surrounding area
Hours: Tuesday - Saturday 11AM - 7 PM

Unity Hospice is a family owned and operated hospice. We are looking for an outstanding admission nurse (RN) to join our team. Duties: What you will do:

Primary duty: Conduct initial assessments and evaluation of new patients; admit them to the hospice program when appropriate
Provide skilled nursing care
Provide case management services
Travel to patients wherever they reside: Home, nursing facility etc.
Participate in an on-call schedule

Requirements: What you need to have:

Valid Registered Nurse (RN) license
Driver’s license, insurance and vehicle
1-2 years of nursing experience (preferred)
Utmost compassion and integrity
