Overview

Join our dedicated team as a Private Duty Registered Nurse (RN) providing exceptional in-home care tailored to meet the unique needs of each patient. This dynamic role offers flexible scheduling options, including shifts from 7AM to 7PM, 7PM to 7AM, and around-the-clock 24/7 coverage. As a vital member of our healthcare family, you will deliver comprehensive nursing services in a compassionate, patient-centered environment, empowering individuals to maintain independence and quality of life within their homes. Your expertise will directly impact patient outcomes by delivering personalized care with enthusiasm and professionalism.

Duties

Provide skilled nursing care in patients’ homes during assigned shifts, ensuring safety and comfort at all times
Administer medications, including injections and IV infusions, following strict aseptic techniques and medical protocols
Conduct thorough patient assessments, vital signs monitoring, and physical examinations to inform ongoing care plans
Manage complex medical procedures such as tracheostomy care, ventilator management, catheterization, and ostomy care
Collaborate with physicians and interdisciplinary teams to develop and update individualized care plans aligned with patients’ needs
Document all patient interactions accurately using electronic health records (EHR) systems like Cerner or Athenahealth while maintaining HIPAA compliance
Respond promptly to emergent situations such as acute pain episodes or trauma incidents, utilizing critical care response procedures
Educate patients and family members on medication administration, wound care, mobility assistance, and disease management strategies
Ensure infection control practices are rigorously followed to prevent hospital-acquired infections in the home setting
Coordinate discharge planning and long-term care strategies for patients transitioning from hospital or assisted living facilities

Skills

Extensive experience working with individuals with developmental disabilities, dementia, or memory care needs
Proficiency in using EMR/EHR systems such as Cerner or eClinicalWorks for accurate documentation and medical records management
Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques including laboratory experience and specimen processing
Skilled in performing advanced procedures such as suturing, tube feeding management, laboratory specimen collection, and sterile processing techniques
Familiarity with critical care environments including ICU or trauma center experience; capable of managing acute pain and emergency response procedures
Ability to operate medical equipment like ventilators, infusion pumps, Hoyer lifts, and telemetry monitors effectively
Knowledge of Workers' Compensation law related to workplace injuries; experience in hospice & palliative medicine preferred
Excellent patient assessment skills combined with compassionate communication tailored for diverse populations including pediatrics and geriatrics
Competence in infection control protocols and aseptic techniques essential for safe home healthcare delivery
Strong organizational skills for managing multiple patients across various shifts while adhering to NCQA standards and ICD coding requirements
