Full Job Description

The Colorado Rural Health Center (CRHC) is seeking a Program Manager with the passion and skills to oversee the Rural Health Transformation Program (RHTP) and collaborate with applicants, partners, and team members and deliver high quality technical assistance for all programs and activities within the RHTP.

CRHC is a non-profit organization that serves as the State Office of Rural Health and has been serving the state of Colorado since 1991. CRHC’s mission is to enhance healthcare services in Colorado by providing information, education, linkages, tools and energy toward addressing rural healthcare issues. By joining our dedicated team, you will be helping a diverse and inclusive statewide constituency of rural healthcare facilities and making an impact by improving access to quality healthcare in Colorado.

As an RHTP Program Manager::

Design and implement a comprehensive technical assistance program that meets organizations where they are, assesses readiness, and delivers actionable support through webinars, trainings, site visits, and resource development.
Guide organizations through grant applications from concept to submission, translating CMS and state requirements, while managing grant tracking, reporting, and compliance to ensure all RHTP and HCPF deliverables are met.
Manage a caseload of organizations at varying stages of readiness, balancing deadlines, and competing priorities with strong follow-through and accountability.
Maintain accurate data in databases, generate reports, and conduct research to produce fact sheets, program materials, and insights that support data-driven decision-making.
Serve as a primary point of contact for applicants, grantees, and partners; collaborate on outreach and marketing efforts; and communicate program updates, resources, and opportunities.
Plan and support workshops, conferences, and site visits, and represent the program at meetings and trainings to strengthen partnerships and advance program goals.

The salary range for this exempt position is $85,000 – $95,000 annually and is commensurate with depth of experience and overall qualifications. CRHC is a Remote-First work environment. This position requires about 30% travel and is funded by HCPF through Rural Health Transformation Funding and is dependent upon that funding for continued employment. First year of the contract is through September 30, 2027, and employment beyond that date is dependent upon receipt of continued funding. Colorado is expected to receive CMS funding for RHTP for a total of five years.

CRHC employees are required to be vaccinated against COVID-19.
