Telehealth Nurse Practitioner (Cardiology) Job Post

Position Summary
We are seeking a highly motivated, compassionate, and skilled Florida-licensed Nurse Practitioner (NP) to join our innovative telehealth team. This role focuses on delivering high-quality, remote post-acute care to patients discharged from acute care settings with complex cardiovascular conditions. The ideal candidate will leverage advanced telecommunication technology to manage care transitions, prevent hospital readmissions, and optimize patient outcomes from the comfort of the patient's home or a skilled nursing facility.

Key Responsibilities

Conduct comprehensive virtual consultations for patients in post-acute care facilities across Florida.
Diagnose, treat, and manage a wide spectrum of cardiovascular conditions, including hypertension, heart failure, arrhythmias, coronary artery disease, and post-cardiac surgery care.
Develop and adjust evidence-based treatment plans, including prescribing medications, lifestyle modifications, and therapeutic interventions.
Review patient records, recent hospital discharge summaries, laboratory data, and imaging studies to formulate comprehensive, facility-appropriate treatment plans.
Collaborate closely with on-site nursing staff, facility medical directors, and primary care providers to ensure seamless transitions of care and effective cardiovascular management.
Provide clinical leadership and guidance for cardiovascular care programs within partner facilities, promoting best practices and optimizing patient outcomes.
Ensure meticulous and timely documentation of all patient encounters within the EMR/EHR system in compliance with all relevant Florida and federal regulations.
Educate facility staff on best practices for cardiovascular care, including prevention strategies, treatment adherence, and patient education.

Position Information

Florida-Licensed Nurse Practitioner (Cardiology) (1099, Remote Telehealth/Post-Acute, RVU-Based)

Location: 100% Remote (Must be licensed in Florida)
Job Type: Independent Contractor (1099)
Schedule: Self-Directed, Highly Flexible Hours (Part-time or Full-time availability)

Qualifications & Requirements

Active Florida Nurse Practitioner License: Must hold a current, unrestricted NP license in the State of Florida (a dealbreaker!).
Education: Master’s or Doctorate degree in Nursing (MSN/DNP) with a specialization in Cardiology or relevant experience in cardiovascular care.
Experience: Minimum of 2 years of clinical experience in cardiology. Previous experience in post-acute care, skilled nursing facilities, or long-term care environments is highly preferred.
Technical Skills: Proficiency in EMR navigation (experience with common SNF EMR systems is a plus) and comfort utilizing telemedicine technology.
Skills: Strong commitment to quality improvement, excellent communication skills for collaborating with diverse facility staff, ability to work autonomously, and clinical empathy tailored to the geriatric and post-acute population.
