Full Job Description

Position Summary

The Registered Nurse (RN) – Skilled Home Health Visiting Nurse is responsible for providing high-quality, patient-centered skilled nursing care to individuals in their homes. This position requires traveling to assigned patient residences to perform comprehensive nursing assessments, skilled treatments, patient and caregiver education, and ongoing clinical evaluations in accordance with physician orders, individualized Plans of Care, and applicable federal, state, and agency regulations.

The RN functions as an independent clinician in the field, utilizing critical thinking and clinical judgment to assess patient needs, identify changes in condition, implement appropriate nursing interventions, and communicate effectively with physicians and the interdisciplinary healthcare team to promote optimal patient outcomes. This position is considered an office position; however, the nurse is expected to perform care and management of patient needs in the home.

Essential Duties and ResponsibilitiesDirect Skilled Nursing Care

Travel to patients' homes throughout the assigned service area to provide skilled nursing visits.
Perform comprehensive Start of Care (SOC), Resumption of Care (ROC), Recertification, Transfer, Discharge, and routine skilled nursing visits.
Conduct complete head-to-toe nursing assessments during each patient encounter.
Assess and monitor changes in physical, psychosocial, and environmental conditions.
Develop, implement, and update individualized Plans of Care in collaboration with the ordering provider.
Administer prescribed medications and injections as ordered.
Perform medication reconciliation and assess medication effectiveness and compliance.
Evaluate pain levels and implement appropriate pain management interventions.
Obtain and document vital signs and other clinical measurements.
Work closely within the office with case manager(s), DON, and other RNs involved to ensure smooth care operations.

Skilled Nursing Procedures

Provide skilled nursing services including, but not limited to:

Complex wound assessment and wound care
Negative Pressure Wound Therapy (Wound VAC) management
Surgical wound care
Pressure injury prevention and treatment
Diabetic ulcer management
Venous and arterial ulcer care
Dressing changes utilizing aseptic technique
Central venous catheter care
PICC line dressing changes
Implanted port care (within agency policy)
Peripheral IV insertion (when applicable)
Intravenous medication administration
Intravenous hydration therapy
IV antibiotic administration
Laboratory specimen collection
Venipuncture
Foley catheter insertion, replacement, irrigation, and maintenance
Suprapubic catheter care
Ostomy assessment and education
Tracheostomy care (if applicable)
Enteral feeding tube assessment and care
Diabetic education and insulin administration
Blood glucose monitoring
Disease process education
Fall risk assessment and prevention
Infection prevention and control education

Patient Assessment

Assess the patient's overall health status at each visit.
Identify actual and potential health problems.
Evaluate the patient's response to prescribed treatments.
Recognize early signs of clinical deterioration.
Notify the physician or appropriate provider of significant changes in patient condition.
Recommend modifications to the Plan of Care when clinically appropriate.
Assess home safety and identify environmental risks.
Evaluate caregiver competency and support needs.

Patient and Caregiver Education

Provide individualized education regarding:

Disease processes
Medication administration
Wound care
IV therapy
Infection prevention
Nutrition and hydration
Fall prevention
Medical equipment use
Signs and symptoms requiring medical attention
Emergency preparedness
Self-management techniques

Evaluate patient and caregiver understanding and reinforce education as needed.

Documentation

Complete all clinical documentation accurately and timely.
Document nursing assessments, interventions, patient response, and education provided.
Maintain compliance with agency documentation standards.
Complete OASIS assessments when required.
Ensure documentation supports medical necessity and skilled nursing services.
Maintain confidentiality in accordance with HIPAA requirements.

Communication

Maintain regular communication with physicians regarding patient status.
Collaborate with therapists, social workers, aides, and other healthcare professionals.
Participate in interdisciplinary care conferences as needed.
Report patient concerns promptly to the Clinical Supervisor or Director of Nursing.
Communicate changes in patient condition immediately when indicated.

Infection Prevention and Safety

Follow Standard Precautions and Transmission-Based Precautions.
Adhere to OSHA Bloodborne Pathogens standards.
Utilize appropriate Personal Protective Equipment (PPE).
Maintain infection prevention practices during all patient encounters.
Safely transport and dispose of medical waste and sharps in accordance with agency policy.

Professional Responsibilities

Maintain current nursing licensure and certifications.
Demonstrate sound clinical judgment and independent decision-making.
Maintain patient confidentiality.
Represent the agency professionally during all home visits.
Participate in orientation, competency validation, continuing education, and required in-service training.
Maintain reliable transportation, a valid driver's license, and automobile insurance as required.
Travel throughout the assigned geographic service area.

Qualifications

Current unrestricted Registered Nurse (RN) license.
Graduate of an accredited school of nursing.
Minimum of one (1) year of nursing experience preferred.
Home health, infusion therapy, wound care, ICU, PACU, Emergency Department, or medical-surgical experience preferred.
Current CPR/BLS certification.
Valid driver's license.
Reliable transportation.
Current automobile insurance.
Ability to travel throughout the assigned service area.
Ability to utilize an Electronic Medical Record (EMR).

Physical Requirements

The Registered Nurse must be able to:

Frequently drive throughout the service area.
Enter patients' homes, including those with stairs or limited accessibility.
Lift, transfer, reposition, push, or pull up to 50 pounds with or without assistance.
Stand, walk, bend, kneel, squat, and reach frequently.
Perform repetitive hand movements required for clinical procedures.
Carry nursing equipment and supplies.
Utilize visual acuity sufficient to perform assessments and medication administration.
Hear and communicate effectively in person and by telephone.
Respond appropriately in emergency situations.
