Identifies primary physician, medical condition (s) requiring skilled HOME HEALTH care, homebound status, medical orders and medications for HOME HEALTH services, and patient/caregiver desire and ability to safely receive home care services. Ensures that the patients geographic location and insurance/ payment source can be managed by the agency. Coordinates with the hospital Case Managers/ discharge planners and equipment companies to ensure a safe discharge to the patients home environment.
