ED Case ManagerSouthern Regional Health System - Riverdale, GAThe Emergency Department Case Manager (ED CM)facilitates the management of the patient?s medical and social needs with emphasis on a specific patient population. He / she is responsible for identifying this population with collaboration with the nursing and medical staff of the Emergency Department. He / she will be responsible for coordinating medical needs and emotional and social foactors impeding the medical plan of care needs of the patient by facilitating disposition into the appropriate level of care. This may include discharge planning and comunity resource referrals. The ED CM uses clincial expertise and problem solving skills to ensure the patient needs are met in the appropriate continuum of care. BSN preferred. 2 years of Case Management experience preferred.
