In accordance with Ministry Home Care policies and procedures collaborates with case manager to provide discipline specific care. Cooperates in and completes the full range of care and documentation responsibilities associated with the discipline. Maintains or exceeds professional practice standards and the professional code of conduct. * Education: Bachelor?s degree in physical therapy from an accredited university * Experience: Minimum 1 year clinical experience * Certification: CPR certification * Licensure: Licensed as a physical therapist by the State of Wisconsin * Valid driver?s license * Other: Use of car and proof of auto insurance required * Skills: Communications/Interpersonal Skills * Full or part time, Monday through Friday.
