Care Manager I - LTCOPhiladelphia Corporation For Aging - Philadelphia, PADEFINITION: Reporting to the Care Management Supervisor and the Nurse Consultant, this position is responsible for the overall care management of a diverse caseload.The care manager facilitates community-based long-term care to disabled consumers age 18 or older, and/or chronically impaired older consumers, and/or their caregivers in their own residence.This position carries responsibility for ongoing care planning, service arrangement, follow-up, and reassessment.
