RN-Case Manager - Home Health (09-129)Sutter Health - Crescent City, CAProvides direct care to patients ranging in age/developmental stage from infants (pediatrics) to geriatrics with the main focus on adults to geriatrics, using the nursing process of assessment, planning, implementation and evaluation. Responsible for managing care of assigned patients and directing and coordinating activities of multidisciplinary team. Demonstrates leadership skills by providing staff teaching, education and skill development. Qualifications Must demonstrate Sutter values of caring, compassion, honesty, integrity, excellence, quality, teamwork and community. BSN preferred. Current CPR.
