Case CoordinatorEdinburg Center - Lexington, MACase Coordinator Full-time (40-hour) position available in an innovative, community-based, step-down program to assist mental health consumers to avoid unnecessary hospitalization or transition successfully from the hospital to community living. Responsibilities include: case coordination, support, transportation, skills training, linkage and outreach. Qualifications: BA in MH related field; at least 2 years experience with adult mental health population; experience in assertive community treatment (ACT) model programs and knowledge of psychosocial rehabilitation preferred.
