This position is responsible for providing patient care patients in the comfort of their own living environment. As the patient/family advocate, and will be performing psychosocial evaluations and assessing and utilizing hospice personnel and community resources for the patient/family. They will act as part of an interdisciplinary team of care to provide the maximum care possible. RJG-2007 Master's Degree in Social Work from a Council on Social Work education accredited graduate school of social work.
