Ministry Health Care - Marshfield, WIPOSITION SUMMARY: The Registered Nurse, working in collaboration with the primary physician, provides coordination of the patient plan of care and the patient care team. The Registered Nurse works with the patient/family/significant other(s) and interdisciplinary team members to develop an individualized plan of care to include educational and discharge planning interventions. Together with interdisciplinary team members, the Registered Nurse evaluates the plan of care and addresses unmet outcomes for further intervention or consultation.
