Physical TherapistVisiting Nurse Health System - Norcross, GAThe primary function of the Physical Therapist is to provide direct care, patient education and the management of the applicable therapy (physical) called for in the patient's treatment plan. Consults with and educates other care team members concerning applicable patient assessment and treatment protocols. Serves as a rehabilitation resource for the care team. EDUCATIONAL REQUIREMENTS: Graduate of an accredited School of Physical Therapy with a Baccalaureate or Master's Degree. LICENSURE: Must have a current licensure by the Georgia State Board of the specified therapy. EXPERIENCE REQUIREMENTS: 2 years in home health who meets the standards for coordination of care and documentation or 5 years clinical experience but not in home health.
