We currently have an opportunity for a full-time Registered Nurse Case Manager who will coordinate resources and services to all patients and promote quality and continuity of care as well as, appropriate utilization of resources. The Case Manager will work in conjunction with nursing, physicians, and ancillary hospital departments and services to assess the acute and post-hospital needs of all patients, infants through geriatric and to coordinate the delivery of services which will allow the patients to function at an optimum level upon discharge.
