Patient Care Coordination A1:Triage patient phone calls and messages, including but not limited, from patients, appointment center, EPIC inbasket or through mychart patient portal, based on program protocol. A2: Serve as patient liaison and follow up with the patients after discussing with the respective treating physician within same day. A3: Track and follow up on lab results reviewed by treating physician and follow up with the patient.
