Clinician III/IV-ICC-FCTCommunity Healthlink - Worcester, MAFamilies and Communities Together (FCT) is a collaboration between Community Healthlink, LUK, Inc., and Communities of Care that oversees three Wraparound Programs operating in the Central MA area as part of the Commonwealth's Children's Behavioral Health Initiative. The FCT Wraparound programs seek to build on family strengths and available support systems to help children with serious emotional disturbance remain in or return to their communities. The FCT staff work with a family to develop a Care Planning Team (CPT), which consists of the child, family, people invited by family, FCT staff, and appropriate state agency and school staff.
