RN Nurse Consultant/Transfer Coordinator
Responsible for providing accurate and efficient telehealth services including, but not limited to, health information, health education, and clinical triage to patients or health plan members of contracted clients. Serves as a health resource person, educator and patient advocate. Upholds the standards of the system-wide Customer Service Program and promotes quality service. Coordinates and facilitates patient transfers into the health system and serves as liaison between hospitals, physicians. Registered Nurse, minimum three years clinical nursing experience required. Must possess superior communication and organizational skills, as well as, exhibit excellent interpersonal skills.
