Job Summary: The Registered Nurse is responsible for the clinical care of assigned home care patients which may include but not be limited to case management of the case, specific nursing tasks, patient assessment, patient / family education, and coordination with the multidisciplinary team including RN, LPN, PT, PTA, OT, COTA, SLP, MSW and Home Health Aides. The RN is responsible to report changes in condition to the physician and to the Clinical Coordinator as appropriate.
