Case ManagerThe case manager serves as the primary liaison for the patient/caregiver, educates patient/caregiver of continuum of care, and facilitates discharge coordination. The case manager communicates to external payers on behalf of patients admitted to the hospital. Acts in partnership with the physician to closely monitor utilization of the patient's resources and works with the patient/family to plan and facilitate a timely discharge to home or to the appropriate level of care while promoting department efficiencies.
