  PURPOSE OF YOUR POSITION: Responsible for the coordination of a multidisciplinary plan of care to ensure the delivery of quality patient care that facilitates the discharge and expected incomes, utilizing appropriate and cost effective resources. GENERAL INFORMATION: Must understand the complexity of the health care system and be able to apply theory with practice in a manner so as to affect the maximum level of communication and rapport with hospital staff and the general public. Must have good leadership skills with effective conflict resolution, motivation, negotiation and problem solving. QUALIFICATIONS: 1. Graduation from an accredited educational program for nursing. 2.
