  Near North Health Service - Chicago, IL

  Facilitate social services required by clients of NNHSC. Maintain active clinic caseload and collaborate with other service programs within the department, as well as, other departments of NNHSC. Maintain a systematized routine for client referrals to the Social Services department, including those clients who require immediate social work intervention. Provide individual patients with the psychosocial support needed to cope with psychosocial stressors and/or medical issues. Assist medical providers by providing supportive social work and therapeutic interventions as required. Assist patient in identifying/obtaining medical resources.
