Physical Therapist PTCatholic Healthcare West - Stockton, CAThe Physical Therapist in Home Care performs patient evaluations, develops treatment plans and sets goals for treatment. Communicates patient status, progress and discharge recommendations to the interdisciplinary team. Oversees the provision of assistants and HOME HEALTH aides as required. This position requires providing optimum home care to a patient population ranging in age from infant through elderly. This position will be employed by St. Jospeh's HOME HEALTH Care and offers a pay-per-visit reimbursement arrangement.
