There are currently 4 advanced practitioners on Interventional Cardiology service; 3 @ 40 hrs and 1 @ 20 hrs with a recently vacated 2nd part-time AP @ 20 hrs.
Looking for 2 years experience as NP with at least 2 years experience as cardiac RN (tele or CCU) or PA with at least 2 year of cardiac experience. Required to take out of hospital overnight call 1 night per week for routine issues by phone.
Must be board certified for advanced practice. ACLS certification preferred.
Responsibilities include :
1. Working with supervising physician in assessment and management of cardiology patients with acute coronary syndromes, congestive heart failure, arrhythmias, hypertension and stable chest pain syndromes, including admission, developing interdisciplinary therapeutic plans of care, and discharge planning.
2. Seeing Cardiology patients in outpatient clinic for hospital follow-up or routine visits under supervision of attending physician.

Responsibilities

Under the supervision of the attending physician, the AHP provides direct care for patients on the CVM Service. In concert with the supervising physician, the AHP is responsible for admissions and pre-cath physicals and obtaining patient consent, responding to questions from the staff concerning CVM patients. The position includes some management of arterial and vascular access sites and discharging of patients.

Essential Duties & Responsibilities including but not limited to:

1) Non-Clinical Functions:
* Manages and communicates information through various processes including but not limited to use of computers, telephone systems, direct and indirect observations and documents in the medical record.
* Completes a quarterly review with the supervising physician on a timely basis.
* Complies with all policies and procedures as set forth by Lahey Clinic and all state and federal regulations.
* Meets all guidelines for continuing education/re-certification for continued practice in the department.
* Assists department in controlling expenses on an on-going basis and identifies areas of potential cost savings.
* Is alert to potential Quality Assurance/Continuous Quality Improvement (CQI) issues and actively participates in the problem solving process.
2) Patient Care Assessment
* Receives and triages telephone request for information, appointments and/or medication renewals. Makes appropriate and timely decisions regarding the patient?s complaint and/or request utilizing clinical knowledge and skill to extract pertinent data.
* Obtains detailed history and perform baseline and interval health examinations. Completes medical histories and physical assessments and differentiates normal results from abnormal findings. Identifies wellness and risk factors and communicate relevant data via dictation and/or provide complete, accurate written documentation in the patient?s medical record.
* Determines and orders necessary laboratory and diagnostic tests. Performs and interprets various diagnostic tests including blood tests, MRI, CT scans, EKGs, etc. Assists patients in areas other than the primary specialty including, but not limited to, recognizing the patient?s mental and physical condition and initiating appropriate assistance from other health care professionals.
* Anticipates potential problems/complications and intervenes before they occur and initiates protocols and therapeutic measures in collaboration with the physician and according to established standards of practice.
3) Patient Care Planning
* Develops and initiates the plan of care by collaborating with physician, other health care professionals and family members. Provides counseling and education to both the patient and the family. Ensures treatment plan is individualized based on the patient and family?s level of understanding.
* Formulates desired outcomes specific to the individual patient problem/diagnosis and specify interventions as appropriate. Prescribes, in collaboration with or at the direction of the attending physician, medications specific to the patient?s diagnosis and level of medication tolerance.
* Reviews the plan of care on an ongoing basis and revise the plan as necessary per patient?s progress or lack thereof. In conjunction with the attending physician, researches medical protocols to determine an appropriate change in care plan.
4) Patient Care Implementation
* Actively involves the patient and family as appropriate in implementing the plan of care. Prioritizes care needs and ensure the appropriate availability and accessibility of treatment for the patient.
* Utilizes physical assessment skills on an ongoing basis to identify, communicate and document patient problems, recognize changes in mental and physical condition. Implements appropriate action based on assessment to ensure patient safety, well-being and satisfaction.
* Anticipates potential emergency situations and perform appropriate treatment/intervention measures. Performs a variety of routine procedures to expedite patient?s response to plan of care.
* Identifies, coordinates and delegates as necessary, appropriate health care professionals to facilitate delivery of care. Provides appropriate and timely communication with the attending physician.
5) Patient Care Evaluation
* Participates in grand rounds, as necessary, to assure continuity and quality of care.
* Receives, triages and evaluates telephone requests for appointments and/or medication renewals.
* Follows through on achievement or lack thereof based on outcome evaluation. Provides appropriate and timely communication with attending physician.
6) Documentation
* Provides thorough and accurate documentation in the medical record by documenting all pertinent data, therapeutic interventions, patient responses and any medical/surgical procedures
* Ensures that summaries include complete examinations, office visits, telephone conversations, and testing requests.
* Manages and communicates information through various processes which include but are not limited to the use of computers, telephone systems, direct and indirect observations and document in the medical record.

Qualifications

1. Education:
Physician Assistant: Graduate of an accredited Physician Program. Documentation of registration for next NCCPA Board administration or successful completion of NCCPA Board and maintainence of appropriate CME
Practitioner RN: Graduate of a Master?s Program in Nursing. Maintain currency through Clinical Education Units.

Licensure, Certification, Registration:
Certification and current license to practice in the Commonwealth of Massachusetts.
ACLS certification.
DEA number and Mass Department of Public Health Division of Food and Drug Restricted Substances registration number as appropriate.
Written prescription privilege protocols signed by all attendings with whom the individual will be working.

Skills, Knowledge & Abilities: In the absence of any of these skills, demonstrates the ability to be thoroughly trained to meet organizational standards.
Good communication skills. Ability to interact effectively with medical staff , nursing staff, and other clinical and administrative colleagues as well as with patients and their families. Knowledge of clinical procedures and protocols. Demonstrated ability to maintain confidentiality and ability to provide appropriate emotional support.

Experience: .
Direct patient care experience (preferably to include in-patient care) prior to graduation and /or clinic experience while in program.

Shift

DAYS, No mandatory weekend or holiday call.
