OVERVIEW: This person will provide a full continuum of chemical dependency counseling and case management in our Aftercare Plus Program. This position will be working with female and male clients, providing both individual and group counseling and case management services. The focus will be on relapse prevention and case management to include: assisting clients with finding housing, employment, education and volunteer opportunities. Schedule is Monday Friday, with the ability to work one evening per week and be flexible for program needs.

RESPONSIBILITIES

* Screening individuals to establish appropriateness and/or eligibility for program.
* Assessment of individual needs and make appropriate referral and service coordination for community-based services.
* Provision of chemical dependency continuing care counseling and case management utilizing techniques that meet professional and ethical standards.
* Facilitation of Continuing Care Recovery Support Groups.
* Provision of counseling and support for clients who are involved in job seeking process or receiving disability benefits in conjunction with the Employment Specialist/s.
* Provision of resources and support for clients who are seeking stable, sober housing in the community
* Coordination of care and services within the RESOURCE Continuum of Care.
* Coordination of care and services with community-based agencies and services
* Maintaining accurate and timely documentation for client files.
* Conducting primary groups, lectures and support groups on an as needed basis.
* Observing and documenting the collection of specimens for urinalysis.
* Active participation in RESOURCE diversity efforts
* Conducting public relations and outreach initiatives to various community agencies
* Attending various meetings required of position and our agency.
* Completion of other duties assigned by supervisor.

QUALIFICATIONS

* BA in human services preferred or equivalent 4 years of experience.
* Completion of Chemical Dependency Counselor Training Program desired
* Must be licensed (LADC) or license eligible to meet the requirements set by the Minnesota Board of Behavioral Health Professionals (BBHT) licensure as a chemical dependency counselor.
* Ability to work as a team member with coworkers and referral sources.
* Experience working with individuals from diverse backgrounds preferred.
* Experience providing services to parole/probation referrals, hospitals, county social workers, and other key stakeholders.
* Capable of independent travel with valid drivers license and insurance desired

SALARY: Depends on qualifications. This position maintains a Monday Friday schedule, the ability to work a flexible schedule to meet program needs.

BENEFITS: Prorated at 75% FTE benefits include paid vacation, sick time and holidays. Full enrollment into medical, dental, life, LTD insurance, and 401(k) plan. Other optional benefits are available.
