We are currently recruiting for Senior Trainers for Clinical Case Management:

The Scope of Work includes but is not limited to:

1. Site Management

a. Stand-up new site which includes i.) Initial site survey: facility, equipment, supplies, etc.; ii.) Screen and approve clinical delivery team hires; iii.) Vendor and other department relationships: food service, IT, etc.; iv.) Marketing and recruitment: hospital, physician, community, etc.

b. Maintain quality of program delivery including:

i.) Quality assurance:

1. Host element specialist meetings (via monthly videoconference)

2. Communicate program updates and modifications

3. Monitor clinical delivery team productivity and effectiveness

4. Obtain and review annual data outcomes summary

5. Site visits as needed

ii.) Oversight of Program Directors/Clinical Delivery Teams

c. Assists the Program Director in efforts to actively market and promote the Program to prospective participants and health care professionals to meet pre-determined participant enrollment objectives.

d. Provides educational in-services as necessary.

e. Facilitates the hiring and training of other nurse case managers.

f. Assists with the recruitment process of potential participants.

g. Coordinates and/or assists with the screening process of potential participants.

h. Coordinates and assists with the medical record review of potential participants for medical eligibility.

i. Assists with the data collection during the screening process and throughout the Program per program protocol.

j. Ability to assist with performing a comprehensive health assessment, including a baseline physical assessment.

k. Assists with efforts to actively market and promote the Program to prospective participants, health care professionals, and health plans in order to meet pre-determined participant enrollment objectives.

l. Participates in nursing, physician, hospital, and community program outreach/marketing activities.

m. Participates in the oversight of interdisciplinary team development of a plan of care for participants.

n. Participates in the development of concurrent program and follow-up of resources.

o. Oversight of clinical outcomes to determine sites are meeting or not meeting Program benchmarks.

p. Oversight of interdisciplinary discharge planning for each participant who completes and/or does not complete the Program.

q. Participates in the Program’s quality management activities.

r. Delivers educational lectures as needed.

s. Provides outreach to hospital, medical, or lay communities as needed.

t. Provides educational in-services as needed.

u. Facilitates the hiring and training of additional nurse/case managers.

v. Assists with reimbursement procedures as outlined by contracted and non-contracted payers.

w. Assists with maintaining the pertinent, ongoing documentation of the health status of each participant throughout the Program.

x. Serves as a role model for all elements of the Program.

y. Facilitates high level of participant satisfaction.

2. Train the Trainers

a. Teach at 3-day training retreats

b. Create/update educational materials and program resources (lectures, demos, etc.)

3. Direct to consumer

a. Moderate community forum (http://feelthelove.ornishspectrum.com...

b. Contribute to newsletters, blogs, articles

c. Speaking engagements

4. Academic/Scientific

a. Maintain/update program delivery materials

b. Draft clinical memos

c. Work with Healthways science department to optimize data collection (methods, procedures, surveys, etc.)

d. Speaking engagements

Additional duties as assigned by the President and/or Director of Lifestyle Programs

Qualifications

LICENSES, EXPERIENCE AND ABILITIES

1. Registered license from the appropriate state board of nursing.

2. Minimum of two years clinical experience in adult cardiovascular nursing and/or cardiac rehabilitation. Previous experience working in an Ornish program is required.

3. Minimum of two years clinical experience in case management and coaching.

4. Formal education/training in cardiac rehabilitation and/or exercise physiology.

5. Current certification in Advanced Cardiac Life Support (ACLS).

6. Exemplary interpersonal communication, time management, and organizational skills.

7. Ability to work in a team environment,

8. Maintains professional certification/licensure.
