PURPOSE To provide coverage for a variety of routine Medical Record Department technical processing functions to include record retrieval and delivery, chart pick up, record processing for completion, assembly, analysis, transcription processing, physician assistance, microfilm processing, release of information, chart locator updates, chart release, chart deficiency, telephone coverage utilizing appropriate customer service skills, and filing. Incumbent may perform only certain of the following responsibilities depending on their particular work assignment.

ESSENTIAL RESPONSIBILITIES
1. Retrieves records based upon printed clinic schedules
2. Identifies emergency room records of upcoming clinic appointments and faxes request list to Inpatient Medical Records at least the day before the appointment
3. Identifies inpatient records of upcoming clinic appointments and faxes request list to Inpatient Medical Records at least the day before the appointment
4. Delivers records to clinics within specified time frames
5. Performs data entry of demographic information into research data base
6. Generates weekly teams lists of undiagnosed patients for physician meetings and provides records as scheduled
7. Records medical record number, patient name, and any other information necessary to retrieve the record accurately and efficiently
8. Records caller name, extension, and department/clinic accurately and efficiently

FORMAL EDUCATION AND EXPERIENCE

Knowledge

Required for completely satisfactory performance in this job is a knowledge of medical record format, computerized registration inquiry process and back-up manual registration system, as well as admissions process. Working knowledge of MR functions to include chart order/assembly, Terminal Digit Order filing and record flow of department. Working knowledge of computerized access systems.

Skills

Required for completely satisfactory performance in this job is the ability to communicate effectively, provide good customer service, problem solve routine medical record issues, prioritize tasks, be punctual and dependable regarding work tasks, work independently, and pay attention to detail. Must utilize well-organized work habits along with good written and verbal communication skills, excellent filing and retrieval skills, utilize electronic messaging, accurate typing/keyboarding skills, and perform accurate data entry/verification/updating.

Formal Education

and Experience

The formal education and/or experience required for completely satisfactory performance in this job are as follows

* High school diploma or the equivalent.
* A minimum of one (1) year of experience in medical record department or equivalent.

The following is preferred

* A minimum of two (2) years of medical record department experience.
