**Summary:**The AMR CPS Practitioner is a unique professional with expertise built on the foundation of a Paramedic enhanced with the additional knowledge and skills necessary to manage patients who utilize multiple sectors of the healthcare system and need a more focused, integrated approach to improve their health, decrease use of the more expensive acute care components of the healthcare system and improve patient and provider experience. The AMR CPS program is designed to coordinate into the existing healthcare structure and facilitate more appropriate use of the system. It is not the intent of the AMR CPS Program to supplant the current system of care.

**_Essential Duties and Responsibilities:_**

* Maintain a professional appearance and demeanor at all times.
* Must have the ability to interact well with physicians, nurses and senior health facility staff.
* Must have the ability to understand complex issues and provide solutions tailored to meet needs of patients and facilities (some medical, some social)
* Occasionally present materials to multiple audiences in multiple venues.
* Time management and planning skills are essential.
* Must have the ability to use and adapt current computer technology.
* Must possess a good bedside manner.
* Must have the ability to coordinate multidisciplinary teams from within and outside of AMR.
* Must have a good understanding of current state of healthcare on a local and national level.
* Must have a good working knowledge of common computer applications, (i.e., PowerPoint, Word and Excel.)
* Demonstrate excellent patient care skills.
* Consult with the patient’s Primary Care Physician regarding patient condition on a regular basis.
* Demonstrate and maintain effective communication skills with personnel in the communications center, patients and patients’ family, and hospital as well as interacting agencies.
* Obtain and maintain all appropriate patient identification data for report completion.
* Assure that CPS vehicle is completely stocked and in good running condition throughout the duty shift.
* Maintain the cleanliness of the vehicle on a consistent basis.
* Consistently meet or exceed current criteria for continuing education.
* Actively participate in the CQI process.
* Responsible for adhering to all company policies and procedures.
* Consistently meet the physical requirements of the position.

# _Non-Essential Duties and Responsibilities:_

* Meet and communicate with existing customers/staff members (including skilled nursing, assisted living and hospital facilities) on a regular basis to ensure that the program is meeting their needs and to determine if there are any additional services they require. Report these interactions to the Operations Manager on a regular basis.
* Participate in community programs to maintain AMR image and establish strong community relations.
* Will play a key role as spokesperson for the program with local news media.
* Perform other duties as assigned.

**_Minimum Qualifications:_**

**Knowledge and Skills:**

* Excellent communication skills (written and oral)
* Must be a self-starter and have the ability to provide excellent customer service with limited supervision

The selection process for this elite group of practitioners must evaluate a slightly different skill set than the traditional emergency or inter-facility transport paramedic. In addition to a solid understanding and competency in the core content of emergency health care, the AMR CPS must clearly understand and be able to develop competency in:

* Evaluation and management of chronic diseases, particularly CHF, Pneumonia and Ischemic Heart Disease
* The appropriate utilization of healthcare resources based on patient needs and the concept of an overarching medical home
* Simple, sustainable approaches to managing patients logistical needs, such as physician appointments, medication reconciliation and appropriate usage, support for appropriate dietary needs, identification of the appropriate actions to take with changes in health status and coordination of all health system components
* Communication with and between primary care physicians, hospitalists, case managers, home health agency staff, specialists, family and the patient
* Identification of early patient deterioration requiring intervention and correction
