SUMMARY The Licensed Practical Nurse (LPN) (a) assists each resident in achieving and maintaining their highest practical physical, mental, and psychosocial well-being through the provision of medication and treatment administration and through quality, personal care; and (b) plans, directs and provides direct care to residents in accordance with nursing practice standards, established policies and procedures, individual plan of resident care and regulatory requirements. PRIMARY RESPONSIBILITIES 1) Perform nursing techniques for the comfort and well-being of the resident. Act to ensure that residents are treated with respect and dignity. a. Follow all shift routines, policies and procedures regarding monitoring of vital signs, weights, and administration of medications and treatments. b. Administer prescribed medications and treatments per doctors' orders. Assist residents with activities as care-planned. c. Administer medical/nursing services, such as catheterization, tube feedings, suctioning, range of motion, applying/changing bandages, packs, colostomies and drainage bags, as needed. d. Assist residents with personal cares including bathing, dressing, grooming and toileting, ensuring that residents are clean, neatly dressed, and well-groomed. e. Answer call lights promptly and determine/meet resident needs. f. Manage challenging behaviors as care-planned, to include reproaching and use of aromatherapy. g. Call physicians to report resident change in condition and document same.
Keep families updated on resident condition and document details. h. Comply with acute emergency protocols and tasks involved.
Follow all DNR/DNI/CPR/Comfort Care (Do Not Resuscitate/Do Not Intubate/Cardio Pulmonary Resuscitation) protocols. i. Supervise dining room. Fill in to serve meals and assist residents with eating, as needed. j. Make rounds to observe residents' physical and psychosocial status. 2) Supervise and oversee nursing/NAR staff. a. Model positive communication skills with NARs (Nursing Assistant Registered), TMAs (Trained Medication Assistants) (if applicable), inter-disciplinary team, customers, residents and families. b. Assist in assigning personnel appropriately to meet resident needs.
Communicate expectations and ensure that unit personnel are carrying out their assignments. c. Participate in performance evaluations of unit staff, as requested by immediate supervisor. 3) Follow safety and sanitation protocol to meet state and federal regulations. a. Initiate all Incident/Accident reports involving residents, staff and visitors. b. Report immediately any equipment malfunction to designated employee. c. Report occupational exposures to needle-sticks, blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policy and procedures. d. Correct, report and anticipate potentially hazardous conditions. e. Report immediately any suspected abuse or neglect per facility policy. 4) Maintain and perform all required documentation in an accurate and timely manner according to policies and procedures of the facility. a. Accurately chart Nurse's Notes reflecting the care provided and the resident's response. b. Complete and update resident care plans, as needed. c. Accurately transcribe Physician's Orders per protocol and forward necessary information to other departments. d. Document use and effectiveness of PRN (as needed) medication administration, in accordance with protocol. e. Initial Medication Administration Record (MAR) after medication is administered per facility policy and procedure. Accurately document refusals for medication on back of MAR. f. Participate in shift reports (end-of-shift reports to other staff to update resident status) in a private area. 5) Serve on, participate in and attend various committees and meetings of the facility to enhance inter-disciplinary process, as assigned. a. Attend designated nursing meetings as scheduled. SECONDARY RESPONSIBILITIES
1. Perform miscellaneous projects and completes various tasks as requested by management.
2. Attend educational offerings and obtain at least 12 hours of continuing education per year per regulation. Requirements JOB SPECIFICATIONS Education, Experience and Credentials
Graduate of an accredited school of nursing required.
Must have and maintain current licensure in the State of Minnesota as an LPN.
One or more years experience in long-term care highly preferred.
Current certification in CPR preferred. Knowledge, Skills and Abilities
Able to prioritize duties and re-approach as necessary.
Able to work under fast-paced and stressful conditions. Able to deal effectively with stress.
Able to communicate effectively with inter-disciplinary team, including Hospice, Social Services, Therapy, Dietary, etc.
Able to maintain privacy and dignity during cares, administration of professional services and administration of medication and treatments.
Able to practice HIPAA compliance regarding all communication and resident records.
Able to follow infection control procedures and safety procedures.
Able to practice conflict resolution between residents, staff, families, community, and other disciplines (i.e. Hospice, Therapy, M.D.s, N.P.s, etc.).
Able to develop and maintain a good working relationship with inter-departmental personnel.
Able to demonstrate a proactive commitment to customer service and performance improvement efforts. Able to demonstrate and communicate a positive attitude.
Able to work as an effective and proactive team-player; understands the importance of supporting the organization, residents/families/customers and other Sholom employees.
Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.
Excellent interpersonal skills to interact effectively with a variety of people and personalities inside and outside the organization. Able to effectively handle interactions at all levels and to respond appropriately in sensitive situations.
Communicate clearly and effectively both orally and in writing to all levels of people inside and outside the organization, including possessing good English speaking skills, fluency and understandability.
Strong computer skills including Microsoft applications such as Word, Excel, and Outlook, and other applicable clinical packages.
Able to assist residents with mobility needs such as transfers, ambulation, escorts or use of assistive devices. Follow established safety protocol and use proper body mechanics. PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB Incumbent must be able to
Perform medium work, with occasional lifting up to 50 pounds.
Push and pull people in wheelchairs weighing up to 150 pounds a distance of several feet.
Hear, speak, and effectively verbally communicate in the English language, including following oral and written instructions, in order to communicate with people inside and outside the organization.
Sit at a desk for extended periods of time and perform long hours of work sitting at a computer.
Move around office area and throughout the facility repeatedly throughout the day.
Apply manual dexterity for computer keyboarding, office and medical equipment use.
Turn, bend, stoop, kneel, squat, and reach frequently in the performance of nursing responsibilities.
Work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed. TOOLS AND EQUIPMENT USED Incumbent must be able to use telephone, copier, computer, calculator, and fax machine, as well as thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, IV equipment, suction machines, nebulizer machines, and oxygen equipment. WORK ENVIRONMENT The work environment is indoors. May be exposed to blood, bodily fluids, and other potentially infectious material.
