Activities associated with the job include reviewing the daily notes and reports completed by staff, providing assistance to clients to reinforce daily living skills, monitoring the use of medications, transporting clients to various functions, and assisting with other daily activities as needed. Incumbents are responsible for the activities of staff and the whereabouts of all clients under their supervision at all times and ensuring that required documentation is completed in an accurate, thorough and timely manner. Incumbents are on call twenty-four hours a day and must respond in a timely manner to unexpected events and/or emergencies with clients under their care. The duties and responsibilities of the position require the ability to effectively supervise, support and direct the activities of staff, considerable patience and understanding when dealing with clients and family members, the ability to react quickly and appropriately to unpredictable circumstances, and strong interpersonal, organizational and decision making skills. Job performance is evaluated by the Supported Living Supervisor through review of work related decisions, ability to interact successfully with clients and their families, ability to effectively supervise and direct the activities of assigned staff, and documentation of daily events and client goals. Job activities are also subject to annual state audits. Illustrative Examples of Work -Schedules support staff to ensure adequate coverage and supervision of clients at all times.-Reviews the daily logs and other notes completed by staff to ensure adequate documentation of clients’ activities and progress toward established goals.-Engages and directs the efforts of assigned staff to teach personal and social skills as indicated in each client’s support plan by providing guidance, reinforcement and motivation.-Meets with clinical teams, doctors, case managers, etc. to discuss client issues and objectives.-Assists with ensuring that assigned staff receive all prescribed training and become familiar with available community resources.-Maintains household operating accounts in accordance with acceptable accounting procedures.-Reports all medical and behavioral incidents to the Operations Manager in a timely manner and completes all necessary documentation.-Teaches, supervises and counsels clients in assigned tasks related to the attainment of goals established in the Individual Support Plan.-Attends staff meetings and Individual Support Plan reviews.-Assists with the administration of prescribed medications and documents medications taken on a daily basis.-Participates in housekeeping duties and ensures that each client maintains appropriate and proper hygiene practices when necessary.-Transports clients to medical appointments, special events and activities, socialization activities, etc.-Assists with daily living skills and interacts with clients in an appropriate manner.-Assists with planning various social and community outings and transports clients as needed.-Engages clients in recreational activities such as games, sports, arts and crafts, etc.-Assists with planning special occasions such as birthdays, holidays, picnics, etc.-Assists with planning menus and the preparation and clean up of meals.-Interacts with clients to determine needs and assist with making progress toward established socialization goals.-Ensures that an adequate inventory of client medications and other household items including cleaning supplies, toiletries, etc. are maintained for each household.-Assists in dealing with emotional outbursts and occasionally unpredictable behavior in a professional and appropriate manner.-Ensures compliance with all reporting requirements related to Incident Management and Abuse/Mistreatment.-Ensures that all records of all assigned programmatic and financial activities including daily logs, expense summaries, progress notes, monthly reports, or other reports or information requested by the supervisor are maintained and submitted in a timely manner.-Performs related duties as required. Necessary Requirements of Work Graduation from an accredited high school or GED program supplemented with additional training in working with mentally and/or physically challenged individuals; experience supervising the activities of support staff; considerable experience working in a supported living setting; ability to interact successfully with special needs clients; strong organizational and decision making skills; or any equivalent combination of education and experience to provide the following knowledge, abilities and skills: -Knowledge of the physical and emotional needs of the developmentally and/or physically disabled population.-Knowledge of proper personal hygiene practices.-Knowledge of the nutritional needs of special needs clients including special dietary supplements.-Knowledge of reporting requirements related to Incident Management and Abuse/Mistreatment.-Knowledge of Universal Precautions, First Aide and CPR.-Knowledge of clients’ rights and the development and application of Individual Support Plans.-Knowledge of commonly used medicines for special needs clients including possible side effects and symptoms of improper usage.-Knowledge of the procedures used to monitor and document the daily activities and progress of clients.-Knowledge of supervisory techniques and practices including the selection, motivation, evaluation and discipline and/or discharge of staff.-Ability to effectively supervise and direct the activities of support staff.-Ability to effectively monitor the safety and activities of clients.-Ability to plan and organize daily activities in accordance with the Individual Support Plan.-Ability to interact effectively with clients and diffuse potentially problematic situations.-Ability to maintain detailed notes, logs, and other reports utilized to document the daily activities of clients.-Ability to effectively plan and coordinate special events and activities for clients.-Ability to respond to emergency and/or unpredictable situations in a calm, patient, and appropriate manner.-Ability to perform the physically demanding aspects of the job and be available for emergencies twenty-four hours per day, seven days per week. Necessary Special Requirements -Possession and maintenance of a current, valid Tennessee or North Carolina Drivers License and the ability to be insured at standard vehicle liability rates.-Participation and completion of all training requirements required by state regulations and Omni Visions, Inc.
