The physical therapist I evaluates, plans, implements and treats a patient to help restore function, according to the principles and practices of physical therapy. This is including, but not limited to, reviewing past and current medical histories, testing, measuring the patient’s strength, range of motion, balance, amount of pain flexibility, endurance, functionality, sensation and reflex status, including dermatome and innervations, coordination, posture, muscle performance, respiration and motor function. Therapists also determine patients’ ability to be independent and reintegrate into the community or workplace. Physical therapists also develop treatment plans and goals with treatment strategy, purpose and outcomes using various modalities, exercises, treatments, interventions and teaching to expedite recovery. Physical therapists document findings and assessments, progress, conduct periodic examinations and modifications as necessary. Demonstrates the ability to meet the needs and provide service to all age groups - infancy through geriatrics. Competent in providing services for the culturally diverse population

EDUCATION, TRAINING, AND EXPERIENCE: MINIMUM REQUIRED:

Graduate from a school approved by the American Physical Therapy Association.

Current registration as a physical therapist.

Current health-care provider CPR certification as a health-care provider.

Ability to establish and maintain effective working relationships with

patients, co-workers, patient families, physicians and a wide variety of

health-care individuals.

Ability to organize work with little supervision.

Good written and communication skills.

PREFERRED:

Recent experience in rehabilitation in an acute-care environment, SNF, physical-rehab unit and/or outpatient setting.

Experience in medical/surgical physical-therapy patient care.

MAJOR DUTIES AND RESPONSIBILITIES:

Demonstrates adherence to the hospital, personnel and department policies and procedures.

Demonstrates satisfactory performance of the competencies in accordance with the established standard policies and procedures of the department.

Provides a safe working environment for patients, visitors, and employees. Assesses risks to patient safety and notifies immediate supervisor so that appropriate precautions can be implemented according to hospital policies and procedures.

Demonstrates accurate and complete documentation of the patient’s condition, and notifies physician when patient's condition warrants intervention.

Acts as patient’s advocate, and maintains patient confidentiality at all times.

Attends staff meetings and participates in the Performance Improvement Program.

Maintains and enhances professional growth and development through seminars, continuing education classes, conferences and professional journals.

Provides assessments, evaluations, plans of care and treatments according to the patient's rehabilitation needs.

Provides professional consultation to the medical staff, patients and family.

Develops, participates and implements staff in services, department and hospital-education programs.

Makes specific work assignments and supervises physical therapists’ assistants, physical-therapy-rehabilitation techs and physical-therapy aides.

Provides instructions in therapeutic procedures to be continued by the patient.

Maintains and cares for equipment and supplies. Documents maintenance and repairs.

Assists in training of physical therapy and physical therapist assistant students.

Maintains established standards of productivity.

ETHICAL BEHAVIOR:

Conducts hospital and department business in an ethical and lawful manner, and willing to report any knowledge of real or potential fraud or abuse according to the LMH Compliance Code of Conduct.

WORKING ENVIRONMENT:

Typical rehabilitation environment for both inpatient and outpatient physical therapy activities. Occasionally travels among facility campuses.

Potential exposure to infectious diseases, blood borne pathogens, and hazardous substance.

PHYSICAL REQUIREMENTS:

Must exhibit extremity mobility and strength to handle objects and people safely in the activities of holding, carrying, pushing, transferring, and pulling 60 pounds. Must be able to lift a minimum of 65 pounds safely.
