The Home Health Registered Nurse is responsible for care planning and primary nursing care for patients assigned to the nurse and directs others in patient care. Will have a working knowledge and practice of the nursing process and demonstrate skill in techniques of good patient care. Serves as a resource and role model for other positions.
The Home Health Registered Nurse is responsible and accountable for the care of the assigned patients and directs others to assure Patient-Centered Care which is based on patient's needs, values, and choices. The nurse is expected to practice autonomously within the scope of professional standards, the rules and regulations established by the Tennessee Board of Nursing, and knowledge of the policies and procedures within Mountain States Health Alliance. He/she has a working knowledge of the nursing process and demonstrates skill in developing and guiding collaborative collegial practice that delivers safe competent care. He/she demonstrates sensitivity and understanding of the diverse cultural backgrounds of our patients and caregivers. The position involves maintaining current knowledge of evidence based nursing practice and the ability to demonstrate critical thinking skills in the implementation of all phases of the nursing process. He/she actively participates in continuous quality/performance improvement of patient care through data collection, as well as, interpreting/evaluating the data and planning actions. The RN serves as a professional role model as a caregiver, mentor, teacher and resource for peers and other providers .
Access to patient information may be necessary to fulfill the requirements of this role; therefore, adherence to MSHA privacy and security policies and practices is a must.
This position may be required to cross train within the agency. Responsibilities in secondary position may require access to medical records and other computer access exceeding routine computer usage. MSHA expects all team members to support the VOS initiative by demonstrating awareness of the VOS system and effectively applying it to his/her work.
Job duties of this position may require access to protected patient information (PHI). The team member will be accountable for appropriate use of the record and compliance with all confidentiality and security policy and procedures related to use, access, and disclosure of PHI.
This position reports to the Skilled Nursing Supervisor. Supervises Licensed Practical Nurses and non-licensed patient care personnel as assigned.
Must be a graduate of an approved School of Nursing. Completion of Health Assessment course from a University or Nursing Education program is desirable. Must be licensed in nursing in the appropriate state . Must have at least 1 year med/surg nursing or comparable experience. Must successfully complete agency orientation. Must have own dependable transportation. Must carry current personal collision and liability insurance on any vehicle driven. Must maintain current driver's license in the appropriate state. Must possess and maintain current certification in cardiopulmonary resuscitation.
The competency and practice of point of care testing is under the direct supervision of the Laboratory Medical Director and/or designee per regulatory rule #1200-6-3-15 Alternate Site Testing, Section (1)(c), 9 of the Tennessee Medical Laboratory Board Division of Health Related Boards. This practice is a collaborative process with the Laboratory and Nursing leaderships.
CONTINUING EDUCATIONAL REQUIREMENTS:
1. Plans and conducts a minimum of two hours of job-related educational programs.
2. Obtains 12 hours educational credit.
3. Verbalizes understanding of Shared Governance Model and willingly serves on at least one Shared Governance, hospital committee, task force, or project to improve patient care or work area.
4. Identifies relevant patient care and/or nursing practice KIQ's (key indicators of quality). This is achieved by using PDCA methodology and the appropriate data collection, aggregation, analysis and trending in the demonstration of measurable improvement outcomes.
5. Attends 75% of staff meetings and reads 100% of the minutes of the meetings missed.
