Previous experience working as a Unit Clerk or a graduate of a Unit Clerk Certificate Program preferred.

PHYSICAL DEMANDS

Ability to read, write, hear and communicate adequately to complete job duties and responsibilities.
Ability to move throughout center without assistance.
Must be able to see adequately enough to read hand written medical records.
DUTIES:

The medical record responsibility includes an analysis and evaluation of in-house medical records:

Quantitatively analyze the medical records to assure completeness, accuracy, and internal consistency while the patient is in the center.
On admission
Periodically thereafter (not less than monthly)
Prepare and assemble the medical record on admission
Prepare labels for chart and room
Complete heading on all forms
Stock necessary forms under the appropriate tabs
Request upon admission and then every two weeks any transfer data not received.
Assist the medical staff and allied health partners in updating the records.
Maintain the flow of reports to the in-house medical records.
Maintain the Overflow File by removing outdated forms from the chart holder at the nursing station.
Arrange the medical record in the proper discharge order upon discharge of the patient.
Forward the medical record to the Health Information Department for further processing upon discharge of the patient.
Assist with MDS system as instructed
The nursing service responsibility includes:

Answer the telephone and relay messages.
Maintain a schedule of patients appointments and assist with arrangements as needed.
Check to see that orders are processed properly:
Fill out missing monthly treatment, medication, or flow records
Notify therapist of orders.
Request laboratory, x-ray and/or special tests and see that reports are received.
Notify all departments of admissions, transfers, and/or discharges.
Transfer medical records, as well as overflow records, to the proper nursing station when a patient is moved from one room to another.
Maintain the inventory of paper and desk supplies.
Turn in charge slips and other requests from the charge nurse to Central Supply Room and replace the supplies.
