**Sign-on bonus of $2,500 with one (1) year commitment paid out as follows:
$625 at hire, $625 at 30 days, $625 at 60 days and $625 at 90 days of employment. **
Description:
The Blended Case Manager supports the physical and behavioral health needs of children and adolescents by identifying strengths and accessing resources that enhance the client and family’s quality of life. The Blended Case Manager advocates for school placements and is knowledgeable about the Special Education system and Individual Placement plans.
The Blended Case Manager collaborates with and monitors other service deliveries to ensure the services are providing what families need. The Blended Case Manager has a monthly productivity goal for services rendered and is held accountable to maintain this goal. This position reports to the Manager of Blended Case Management Services.
Responsibilities:
- Support the physical and behavioral health needs of children and adolescents by identifying strengths and accessing resources that enhance the client and family’s quality of life.
- Advocate for school placements and is knowledgeable about the Special Education system and Individual Placement plans.
- Collaborate with and monitor other service deliveries to ensure the services are providing what families need.
- Work non-traditional hours and provide rotation on-call services.

Requirements:
- Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education.
OR
- Registered nurse: or,
- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences and two years experience in direct contact with mental health consumers.
Additional Requirements:
- Must have a valid driver’s license, current insurance coverage and consistent access to a reliable vehicle.
- Act 33 (PA Child Abuse) must be received prior to hire.
- Act 34 Clearance (PA State Police Record Check) OR the Department of Human Services FBI Fingerprinting must also be received prior to hire along with the Act 33 clearance.
- Mandated Reporter and Safety Care Training within 30 days of hire.
