JOB SUMMARY/ESSENTIAL JOB FUNCTIONS:
Youth Resource Support Specialist will provide services focused on promoting a healthier youth and family environment. This position will also assist youth and families to connect with supportive services through screening and referrals.

DUTIES AND RESPONSIBILITIES:
Screens youth and families for eligibility and determine options for securing and maintaining resources to support those treatment needs.
Refers participates to appropriate resources and community programs as needed.
Assists youth and families to maximum benefits of program services.
Provides youth and families with wrap around services determined essential to successful linkage to resources and services.
Meets with participants in the community or school.
Provides follow-up to the youths, families, and agencies.
Maintains caseload of youth and families that have been assessed and receiving support.
Documents notes in the case management system.
Completes reports and assist in fulfilling contract obligations including outcome tracking.
Responds to potential clients, schools, and officers questions via phone or email concerning the center and the services/resources provided.
Other duties as assigned.

Qualifications

MINIMUM REQUIREMENTS:
EDUCATION: Bachelor’s Degree Social Services, Criminal Justice, or closely related field

EXPERIENCE: One (1) year experience working with children serving and/or justice related systems in the community and a bachelor’s degree in social services, criminal justice, or closely related field. (TRANSCRIPT REQUIRED)
