A hospice social worker starts by evaluating the patient’s physical and emotional condition, needs, resources and family situation. She also considers the goals of the patient and his family. For example, if the patient wants to stay in his own home and retain as much independence as possible, the social worker may arrange for meal delivery services or enlist a home health aide. She also determines what medical services the patient requires. For example, she may suggest physical therapy for a senior patient struggling with impaired mobility. In addition, she often provides counseling services for both the patient and his family.
