It is the objective of Tabula Rasa HealthCare (TRHC) to be the nation?s leader in providing medication risk mitigation services. The Enhanced Medication Therapy Management (EMTM) Community Pharmacist Team Lead helps to carry out this objective by overseeing the community pharmacy network and utilizing his or her clinical knowledge and skills coupled with innovative technology to optimize medication regimens for patients by identifying and rectifying medication-related problems, empowering patients to actively participate in their own clinical decision-making, and collaborating with physicians, pharmacists and other healthcare providers to improve patient outcomes.

EMTM Community Pharmacist

Supports and strengthens TRHC client relationships by supporting community pharmacy network

Supports and assists ECPN Pharmacist Lead in local pharmacy responsibilities

Tracks and reports community pharmacy visits

Assists coordination of educational sessions and training for community pharmacy on TRHC Operations specific but not limited to MedWiseAdvisor (MWA?)

Coordinates training client staff while also working with internal IT to maintain training deliverables

Serves as a liaison for Customer Service and Quality Assurance and makes suggestions to improve TRHC?s service offerings

Documents all client interaction (visits, educational phone calls, training, conference call, e-mails, report and disaster preparedness communication)

Monitors, mentors and trains EMTM pharmacists in a community pharmacy environment.

Monitors pharmacists? performance and team dynamics in relation to EMTM deliverables and program-specific goals, and recognizes the need to escalate employee relations or performance issues beyond verbal conversations (re: productivity, errors, etc.) to PIC, VP and/or HR.

Handles escalated challenges, concerns or requests related to the EMTM program and knows when to escalate to VP and Lead level.

Assists with staff professional development and serves as a train-the-trainer.

Produces and supplies reports as requested.

Monitors the local environment for privacy, use of technology and appropriate tools to complete the tasks

Perform administrative duties (i.e. complete MWA User Access Forms, complete and submit W9 forms, etc)

Healthcare Provider Collaborating

Works with community-based pharmacists to provide EMTM services for enrolled patients.

Works prospectively with TRHC departments (QA, Finance, Operations, etc.) to ensure successful transitions for new clients

Monitors and follows newly implemented clients with weekly phone calls or as needed during first year of service

Meets with all newly transitioned clients on-site with Implementation for go-live

Works with patients? insurance providers (e.g., ClearStone Solutions) if necessary

Works with community pharmacies to educate prescribers and develop an integrated healthcare team Education:

Bachelor’s degree in pharmacy required, PharmD preferred.

Licensing/Certification:
Completed Medication Therapy Management Services certificate program, or within 3 months post hire

Certified Geriatric Pharmacist (CGP) certification obtained within 6 months post hire or eligibility.

Experience & Training:
EMTM experience preferred.
One year or more years of team managing / supervising experience preferred.

Specific Skills & Abilities:
Above-average knowledge of geriatric pharmacotherapy and ability to apply knowledge to clinical practice.
Ability to assess patients, using verbal and visual cues.
Ability to effectively and efficiently identify and rectify medication-related problems.
Excellent verbal and written communication skills with patients, including the ability to express empathy and use reflective listening.
EMTM experience
One year or more years of team managing / supervising experience
Remote/telecommuting work required. On-site and remote/telecommuting work may involve split shifts. On-call and weekend work may be required. Required to travel to off-site location(s) as necessary. Position will require overnight travel and flexibility to adjust travel according to demand.
