Mendota Health is a leading provider of mobile wound care services. We specialize in helping patients heal their wounds by providing the best possible treatment options in the comfort of their own home. This is an excellent opportunity for an experienced NP or PA to join a growing organization whose mission is to change how wound care treatment is delivered and increase access to high quality wound care.
This position a is full-time salaried position with the flexibility to work Monday-Friday. No weekends, evenings, on-call or holidays!
Daily local travel is required to the contracted long-term care facilities. All mileage is reimbursed. Equipment, medical supplies, and training will be provided.
Qualifications
- Graduate of an accredited nurse practitioner or physician assistant program
- A clinical background in adult, family, geriatric, podiatry, or vascular specialties
- At least one year as an advanced practice provider
- Board Certified/Prepared through AANP, ANCC or NCCPA
- Wound care experience a plus
- Nurse Practitioner national certification as ANP, FNP, or GNP required or ability to obtain
- Current, unrestricted license in the state of assignment or the ability to obtain license
- Ability to obtain and/or maintain State DEA licensure / Prescriptive Authority post - where applicable
- Microsoft office and CRM experience preferred
Practice Details
- The PA or NP is an experienced member of the health care team who provides advanced wound care to patients in their home setting at the long-term care facility that they reside.
- Assesses health status for wound care patient populations using advanced practice knowledge and skills.
- Develops and implements an individualized patient plan of care including coordination of care between visits.
- Thorough Assessment: Conduct comprehensive patient assessments, paying special attention to factors that increase the risk of wound formation.
- Advanced treatments like surgical debridement, UltraMIST therapy, and skin substitutes.
- Medication Management: Order necessary medications and provide clear instructions for their use. (Work with LTC staff to ensure necessary medications are ordered and provide clear instructions for their use)
- Educate and Communicate: Educate nursing staff, and maintain open communication with primary care physicians, families, and clinical teams as required.
- Contribute to effective and efficient practice management policies and practices that support patient access to medical care.
- Proven ability to function at highest level within scope of practice.
- Patient education and pro-active disease management experience.
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level of population served.
- This individual must show commitment to exemplary patient care.
