This position is heavy on patient communication via telephone; there is not a great deal of in-person patient interaction. Please take this into consideration before applying. There is also a heavy amount of computer work involved, as a large amount of the duties are completed through the practice’s Electronic Medical Record system. The candidate that will be a best fit for this position will enjoy patient coordination of care, as triaging, and care management, as well as administrative duties, but not require regular in-person patient contact to feel job satisfaction. Our staff and providers rely heavily on our Office Nurse Team for daily support; they truly are invaluable members of our patient care team.

Patient education via phone calls to communicate physician instructions regarding; medication changes, requested diagnostic follow-up/labs, diet changes, next visit, etc.
Assist with patient referrals and coordination of care regarding dialysis, transplant, access, and dietary services.
Maintain contact with patients to ensure anticipated therapeutic objectives are being met (i.e. weight, BP, diet, lab work) and communicate with treating Nephrologists regarding objectives.
Assist physicians with Electronic Medical Record (EMR) system by annotating physicians completely on all interactions and patient care, and in reviewing the electronic 'in-boxes' for physicians when they are out of office; this includes reviewing lab work, test results, etc. and taking any necessary action with the patients to ensure timely care.
Responsible for office medication prior-authorizations and test and procedure prior-authorizations
Triage labs and diagnostic results
Enter, review, refill and maintenance of medications in EMR
Medication pre-authorizations for both office and dialysis patients
Navigation of CKD patients through our practice’s pathway of renal care
EMR software data entry and maintenance of patient’s electronic medical records
Answering and triage phone calls from patients seeking advice/clarification
Cross coverage of other duties as needed

Required Experience:Essential Skills and Experience:
Graduate from an accredited school of nursing; BSN preferred; only exemplary ADNs will be considered
Colorado State RN license in good standing
Current BLS certification
Minimum 3 years of current nursing experience in a correlated field required (med/surg, IM, critical care, cardiology, urology).
Skilled with prioritizing and multitasking; excellent critical thinking
Nephrology/dialysis experience a plus but not required
Office-nursing experience (including heavy triaging/navigating chronic disease) preferred, not required
Knowledge of various lab panels: BMP/CBC/RFP/LFT, etc.
Knowledge of Fluid/Electrolyte and Acid/Base physiology
Communicate verbally and in writing in a precise, clear, and professional manner
Compassion for patients and the desire to positively contribute to their health status
Possess and display a high level of patience and professionalism in all situations
Experience with using an EMR system for patient health record maintenance and patient communication
Possess an intermediate skill level with Microsoft Word, Excel, and Outlook, and website navigation
Ability to function at a high level of efficiency without supervision
Highly motivated self-starter and lifelong-Learner, with a strong work ethic

Possess the skills to work with others in a cooperative and supportive manner
