Position Title: Job Coach Location: NYC -Queens, Manhattan, Brooklyn, and The Bronx
Reports To: Supervisor/Program Manager Cost Center: 54513
General Purpose: In a few brief sentences summarize the primary duties and responsibilities.
Provide Quality Assurance. Provide individual assistance and guidance to consumers with developmental disability/mental retardation/mental illness/visual impairment or other major disability with the mission of obtaining and retaining employment.
Essential Functions: Describe the intrinsic job duties that define the core responsibilities of the job.
Employer site visits to maintain quality of service
Assist in auditing folders monthly
Works effectively with consumers in order to obtain/maintain employment
Maintain charts in a timely manner and audit ready condition
Write clear and concise case notes and service plans in a timely manner.
Work in a cooperative manner with other Goodwill and non-GW programs.
Effective Job Coaches will be able to develop good relationships with consumers, parents, therapists, group homes and employers, in order to facilitate positive movement toward employment goals.
Through those relationships help consumers, to remove obstacles and barriers toward employment goals.
Work effectively with employers in order to facilitate accommodations and address workplace problems/issues to result in greater job retention.
Needs to maintain contact with consumers and employers to identify and resolve problems early on in order to prevent a job loss or termination from the program
Good interpersonal skills
Flexible Schedule and Travel throughout 5 boroughs
Assists consumers in achieving the highest level of independent functioning on the job
Qualifications/Basic Job Requirements: Describe the skills, knowledge and abilities required. Include minimum educational and experience requirements and preferences.
Minimum preferred: Associates degree.
Preferred: 1+ year of experience with the disabled population
Scope of Responsibility & Positions Supervised: Where applicable describes level of authority, decision making discretion, consequences of actions, supervisory and fiscal responsibility. Provide metrics where possible. Level of supervision received.
Special Working Conditions: Describe unusual working conditions or environmental factors.
Job primarily consists of field on-site and off site face to face visits with consumers.
Expected to make site visits during consumer work schedules and will consist of night and weekend work in the community.
