Job Summary: The Clinical Coordinator delivers/oversees care provided to patients as part of the LVPG care delivery team. Integral member of the practice leadership team; focusing on clinical support staff. Serves as a clinical resource for all practice staff, including Practice Director/Manager and Practice Lead. Accountable for clinical staff orientation, education, clinical skills competency, clinical office operations, and clinical skills remediation, as necessary. Facilitates seamless transitions of care across the continuum. Facilitates completion of initiatives designed to enhance the patient care experience, inclusive of pre-visit planning and clinical intelligence tools.
Minimum Requirements: Graduate of an accredited school of nursing as a Registered Nurse (RN).
Three (3) years recent acute care clinical nursing experience.
Demonstrates initiative and is able to function independently as well as collaboratively. Demonstrates the ability to initiate, accept, and adapt to change. Demonstrates time management and organizational skills. Accepts and recognizes differences among people. Excellent computer skills, including electronic medical record (EMR), word processing (Microsoft Word), PowerPoint, and Excel. Demonstrates strong communication skills and mature emotional intelligence. Demonstrates project management skills.
Preferred Qualifications: Bachelor's of Science degree in Nursing (BSN).
Minimum of three (3) years recent experience in a physician office practice setting.
Active member of a professional nursing organization. Professional nursing certification.
Licensure and Certifications: Current Pennsylvania Registered Nurse (RN) license. Current BLS (provided during LVPG Orientation).
