Exhibits a deep understanding of public health and knowledge of the role that the community-owned hospital plays in addressing community health issues. Carries out activities related to community benefit program reporting and evaluation. Researches and utilizes proven evaluation tools designed for community health programming. Adheres to requirements set forth by the Indiana Department of Health and the Internal Revenue Service (IRS) for reporting community benefit as a not-for-profit health system. Works with all aspects of the hospital's community health improvement plan which explains how community health needs (identified through the triennial community health needs assessment) will be addressed by the hospital and partnering community organizations. Serves as a liaison to community health improvement staff located at the community hospitals related to the aforementioned activities.
Bachelor's Degree in Public Health, Public Policy, Social Work, Business or Communications.
Valid Driver's License.
Minimum of 3 years of experience in public/community health or related field. Experience with data analysis software preferred.
Familiarity of health needs of "at-risk" populations, the social determinants of health, and skills related to community health improvement outreach program evaluation. Must have a passion for helping those in need and building a healthier community. Must possess enthusiasm for working with others, the spirit of teamwork, good people and communication skills in order to develop strong internal and external working relationships in order to accomplish program goals. Exhibits resiliency when facing barriers.
