Position Summary: The Nursing Home Transition INTERN is responsible for observing and learning what a Nursing Home Transition Coordinator does to assist a consumer in becoming independent. The NHT Intern will work collaboratively with numerous staff in the NHT program and will observe contact with social service agencies, family members, nursing home staff, Ombudsmen and therapists to successfully plan and organize transition.

Observes AIMs compliance policies.

The Nursing Home Transition INTERN will :
Learn what the Nursing Home Transition Program is and how it benefits the consumers.
Take direction and learn from the LTSS Manager for NHT on how the program works.
Observe the interactions of the NHT Coordinator with the consumer, social workers, families and other agencies.
Ensure they are able to interact with the LTSS Manger to engage in a learning conversation to ensure learning is occurring.
Be support to the NHT Coordinators to learn and understand the process, completion of forms, phone calls, and other tasks to ensure a successful discharge of the consumer.
Travel is required to reasonable geographic areas
Must have a valid PA drivers license in order to provide services to consumers in their home or community.
Personal vehicle must be insured and registered.
Shall function as part of a team, working cooperatively with LTSS Manager and NHT Coordinators.
Have excellent verbal and written communication skills.
Basic computer skills with knowledge of Word, Excel, and data entry using Web-based systems.
Education/Relevant Work Experience: Must be actively enrolled in an approved college or university pursing a Bachelors degree in Human Services or other relevant field of study. Some experience in social work, counseling or aiding individuals and/or families requiring social service assistance is helpful.
The NHT Intern work week is 15 hours. This position is unpaid. Typically, the schedule is Tuesday and Thursdays 8:30am-4:30pm with a 30 minutes unpaid meal break.
Travel to meet with consumers and outside business contacts is required.

Mental Demands: Stressful due to time constraints and multiple tasks//procedures occurring at the same time.
Work environment: The noise level in the work environment is usually moderate.
Manual Dexterity Required: Must be able to manipulate small electronic parts and controls.
