The Director, VBC Clinical Models and Care Redesign , is a highly valued senior clinical leader who brings clinical expertise and discipline to naviHealth’s care redesign and clinical model innovation efforts. A skilled and experienced nurse leader, this individual ensures that naviHealth serves its first core value, that we prioritize the people we serve, by researching and incorporating the most recent, evidence-based practices to the naviHealth clinical model for BPCI Advanced and for designing and improving other clinical services models based on leading scholarship and clinical best practices. He or she serves as a clinical compass to the entire enterprise partnering with other clinical and non-clinical leaders and strategists to transform the way naviHealth delivers value-based care. A successful applicant is a deeply passionate clinician, committed to the continuous improvement of our clinical model, with subject-matter expertise and the ability to translate clinical knowledge to empower others.
What you will be accountable for….
Serve as a subject-matter expert on care protocols and best practice for chronic and medically complex conditions, especially those experienced by a geriatric population
Conduct research and literature reviews to identify relevant evidence-based practices and clinical scholarship to inform strategic changes to naviHealth’s clinical models to promote safe, high-quality, and cost-effective transitions of care
Analyze clinical outcomes and clinical quality information to improve and evolve naviHealth clinical models for VBC programs (e.g., BPCI, ACO), partnering with Quality and Process Improvement to ensure variation and pilots are clinically appropriate
Apply research and clinical expertise to the development of clinical strategies designed to improve outcomes in targeted clinical or medical areas or for specific patient populations
Collaborates with Education colleagues on clinical colleague training (e.g., orientation, clinical coaching, and ongoing development); informs content development and learning methods (e.g., internal “Grand Rounds”) related to complex medical conditions or cases
Provide input and subject-matter expertise as a senior clinical leader to other departments (e.g., Product, Data Science) to drive the clinical appropriateness of projects or to provide a clinical perspective to strengthen market position or investments (e.g., reviewing new CMS models or policies to assess impact on patient outcomes)
Attend relevant conferences, strengthening naviHealth’s relationships and clinical brand in PAC and VBC; deliver presentations or contribute to published thought leadership, as opportunities arise

What you will need to be successful……
Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN)
Advanced degree in nursing (e.g., Master’s or PhD) from a nursing-accredited CNS program; board certification as a CNS, or progress toward board certification, preferred
At least 5 years of clinical nursing experience
Clinical experience with patient populations that include those with chronic and medically complex conditions (e.g., CHF, COPD, Sepsis), those with life-limiting illnesses, and those with post-acute care needs, including rehabilitative as well as palliative care and/or hospice
Knowledge of Medicare, value-based care programs (e.g., BPCI Advanced, ACOs), payors, and knowledge of programs and alternative models for geriatric or dual-eligible populations
Experience with transitional care management and/or case management and an understanding of the post-acute care continuum, including transitions from acute to SNF
Experience with using evidence-based medicine and guidelines for care improvement and redesign, including research, synthesis, and application to naviHealth clinical models
Strong and clear communications skills, with the ability to translate technical or clinical knowledge so that non-clinical stakeholders and partners can understand and apply
Demonstrated leadership skills, with an ability to collaborate with a wide range of stakeholders, including C-suite executives and clients
Proactive approach and self-motivation to review new and emerging practices applicable to the patients naviHealth serves
Flexibility and adaptability in applying expertise and effort to evolving VBC programs
Ability to travel for client meetings or industry events, as needed
