St. David's South Austin Medical Center is an acute care facility with 316 beds, offering a range of complex specialties and sub-specialties, including a nationally accredited oncology program with the area’s only adult bone marrow transplant program; a trauma program that includes all of the capabilities and programmatic elements of a Level II trauma center to treat the most severely injured and critical patients; a comprehensive cardiac program; full-service maternity and newborn care with Level I and II nurseries; and two full-service emergency centers in the communities of Bee Cave and Bastrop.
The Quality Coordinator is responsible for the coordination of systems for collection, analysis, and reporting of data to support Quality Management objectives. This includes but is not limited to organizational and department-specific process improvement activities. The coordinator must be able to develop and maintain effective interpersonal relations, maintain confidentiality of all data, information, and activities, and be able to perform detailed, concentrated work with limited supervision. The QO Coordinator must also be able to analyze data, prioritize projects, and demonstrate ability to develop and report analysis of outcomes.
Coordinates the design and implementation of data collection, and the analysis and presentation of data from a variety of sources to support QM department and facility initiatives. Such collection may include surveys, audits or chart reviews, computer queries or literature searches.
Provides for data analysis including but not limited to: outcome, variance, practice patterns and identification of trends or deviations from expected.
Develops and coordinates accurate and timely data reports
Maintain databases and review sheets as required.
Meet deadlines and timeframes for reviews and reporting.
Collaborate and educate physicians and multidisciplinary hospital staff to facilitate care management to achieve compliance in core measures and improve quality outcomes
Demonstrates ability to perform and coordinate clinical chart abstractions and reviews, both concurrently and/or retrospectively based on the clinical indicator and/or cases referred for investigation, etc. as required or requested. Oversee quality systems analyses, including at least the following, in a timely manner to meet review and reporting requirements:
CMS/JCAHO Core Measures
Oversight of the Perinatal core measures to ensure appropriate abstraction, reporting
Perform targeted/focused reviews and analyses on potential or actual fallouts
Identifies possible solutions / education for fallouts and/or trends identified
Clinical intervention and education of physicians regarding documentation requirements on orders, notes, etc. to achieve compliance.
Collaboration and education of both nursing staff and physicians facilitating care management and discharge planning to achieve compliance.
Collaboration with clinical educators and other staff to provide education and serve as a resource regarding requirements.
OPPE / FPPE reviews as assigned
Assists Medical Staff Departments and Divisions in providing required data and/or information pertinent to medical staff appointments / re-appointments.
Maintains strict confidentiality of all data and information.
Committee and/or meeting participation:
Participates in assigned hospital committee / team meetings to present quality data, information, and/or education.
Participates in assigned Partnership committee / meetings
Participates in assigned CATRAC / Mission Lifeline meetings
Comply with all hospital policies and procedures, safety requirements, and emergency preparedness requirements.
Demonstrates understanding of HCA’s and “Patients First” safety initiative by strict compliance to all safety protocols and procedures.
LEVEL OF FORMAL EDUCATION REQUIRED
Graduate of an accredited school of nursing; BSN preferred.
Licensed as a Registered Nurse in the state of Texas.
Minimum 3 years medical/surgical nursing experience in an acute care facility; performance improvement experience preferred.
Moderate to advanced computer skills, including the use of Microsoft Office (Word, Excel, PowerPoint). Ability to develop, maintain and operate database systems as required to perform quality outcome duties.
Ability to communicate effectively both verbally and in writing. Capability of interpreting data and creating understandable information.
Organize, prioritize and manage time efficiently to meet deadlines.
