Full Job Description
To provide diagnostic screenings and assessment of seniors attending a daily activity site; individual and group therapy to support their treatment needs; outreach to families and referrals to other community services as needed.
Demonstrate motivation and competency in working with geriatric patients.
Conduct a bio-psycho-social assessment of seniors volunteering for support. The assessment is documented in an electronic health record according to department policies.
Develop a treatment plan in accordance with department policies outlining goals and interventions for therapy and coordination of care.
Conduct individual and group therapy sessions with seniors to meet the goals in their treatment plans.
Conduct psycho-education groups with all seniors highlighting the importance of self-care identification of behavioral health concerns mindfulness etc.
Write progress notes in accordance with department policies using of DAP format.
With permission outreach to families to gather information and alert them to concerns.
Outreach to primary care physicians to coordinate care.
Assist seniors in connecting to resources to support their independence and self-care.
Conduct home visits as appropriate to assess living conditions
Conducts risk assessments for potential client harm to self or others.
Be familiar with elder abuse reporting requirements and assess seniors for potential abusive situations.
Consult with geriatric cognitive specialists to better differentiate and treat depression and anxiety that may be mitigated by cognitive challenges.
Participate in workshops and trainings as necessary to keep current in job increasing knowledge of diagnostic and treatment strategies that are considered evidence-based strategies with the geriatric population.
Collaborate with other programs to improve the behavioral health care of seniors including participating on committees.
Receive supervision from a licensed clinician.
Other duties and responsibilities as assigned or needed.
A minimum of a Master's Degree in social work (MSW) from an accredited school.
LCSW with intent to earn clinical supervision hours to obtain LICSW.
Training and/or experience in providing mental health services to a geriatric population.
Must apply and successfully obtain Rhode Island licensure during appropriate timetable.
NECESSARY SPECIAL REQUIREMENTS:
Available during morning hours when Senior Center is open.
Knowledge of community resources for seniors.
JOB KNOWLEDGE SKILL AND ABILITY:
Knowledge and understanding of mental health issues and substance abuse.
Knowledge and ability to use different treatment modalities as needed such as crisis intervention family therapy and group skills.
Knowledge of community agencies for successful referrals and treatment planning.
Ability to organize time and schedule.
Interpersonal skills developed to a level that will allow the smooth cooperation at all levels within the organization.
Ability to successfully complete all required training and certifications
Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.
Ability to add subtract multiply and divide in all unites of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
Uses and accesses information in clients health information record or about a client only as necessary to actual duties and then only in accordance with access levels assigned to this position by Gateway Healthcare Inc. from time-to-time and in accordance with Gateway Healthcare Inc. policies rules and procedures as adopted by Gateway Healthcare Inc. from time-to-time.
Comply with agency ethics and codes of conduct in all contacts with clients co-workers and the community
Observe all agency policies and procedures.
Must be able to appreciate the sensitive/private nature and legal status of information about individual clients obtained maintained or used by Gateway Healthcare Inc. and to not access use disclose or request such information about a client unless necessary to the performance of his/her job.

The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision distance vision peripheral vision depth perception and the abilities to adjust focus. The employee occasionally works in outside weather conditions.
The noise level in the work environment is usually moderate.
